Email all items listed below to UltraLightVideos@pediped.com
(please include all items in a single email)

1. Your 30 second or less digital video in H.264, MP4, MPG, AVI, MOV, or WMV format
2. Your full name, date of birth, and address
3. Your child’s full name and date of birth
4. Your daytime telephone number
5. The following statements:

• "I am 18 years of age or older and a legal resident of and currently residing in one of the 50 United States or D.C."
• "I have read and agree to the pediped Ultra Light Athletic Official Rules."
• "I authorize pediped to contact me at the phone number, street, and e-mail address contained in this email for any
